Waivers

Liability Waiver:

Participation in physical activity may involve risk of personal injury.  I understand and am aware that strength, flexibility and other exercise, including the use of equipment, is a potentially hazardous activity.  I also understand and accept that fitness activities involve risk of injury and even death, and that I am voluntarily participating in these activities, and using equipment with knowledge of the dangers involved.  

I hereby agree to expressly assume and accept all risks of injury, including death in consideration of voluntary participation in the activities and programs of Union Studio Yoga and to use its facilities and equipment.  I do hereby waive and release Union Studio Yoga and their officers, agents, employees, representatives and all others from any and all responsibilities or liability for injury or damages resulting from my participation in any activities or my use of equipment in Union Studio Yoga, or arising out of my participation in any activities at said facility.

I understand participation is strictly voluntary, and that I assume the risk for harm or injuries caused by such participation.  I have been strongly advised that I have sufficient insurance coverage.  I will report all accidents as soon as possible to the instructor and management.  It is recommended that I consult with a  physician prior to participation in any physical activity, I hereby declare myself to be physically sound and suffering from no condition, impairment, disease, infirmity, or other illness that would prevent my participation in any of the activities and programs of Union Studio Yoga, and /or use of facilities, equipment or machinery except as hereinafter stated. 

I do hereby acknowledge that I have been informed of the recommendation for a physician's approval prior to my voluntary participation in any exercise/fitness activity.  I also acknowledge that it has been recommended that I have yearly, or more frequent, physical examination and consultation with my personal physician as to physical activity, exercise, and use of facilities and/or equipment so that I might have recommendations concerning the usage of these activities and equipment.  I acknowledge that I have either had a physical examination and have been given my physician's permission to participate, or that I have decided to participate in without that approval of my physician and do hereby assume all responsibility for my actions and physical conditions arising from any participation in any exercise/fitness activity, and /or use of facilities and equipment at Union Studio Yoga.

Photo Release:

I also acknowledge that by signing this waiver I give Union Studio Yoga permission to photograph classes and students (possibly you) for social media marketing and promotion.

By this Waiver, I assume any risk, and take full responsibility and waive any claims of personal injury, death or damage to personal property associated with Union Studio Yoga, including but not limited to receiving yoga training at the facility, using the facility and its equipment in any manner, form or fashion, and practicing and/or engaging in yoga activities or other related activities on and off the premises.
